Pharmacology
1. Pharmacodynamics:
a.) Telmisartan
It is a competitive antagonist of angiotensin-2 atAT-1 receptor. It decreases peripheral resistance and lowers BP in hypertensiveindividuals. It blocks all overt actions of angiotensin-2 such as: 1.Vasoconstriction, 2. Central & Peripheral sympathetic stimulus, 3. Releaseof aldosterone and adrenaline, 4. Salt and water reabsorption, 5. Centralactions like thirst, vasopressin release, and growth promoting actions on heartand blood vessels.
b.) Amlodipine
Amlodipine is a second generation dihydropyridineCa channel blocker. It exerts it`s antihypertensive, antianginal actionsthrough blocking the influx of Ca ions through voltage gated L-type Ca channelsto the peripheral vascular smooth muscle cells, Coronary smooth muscle cellsand to the myocardial cells. Thus causes dilatation of vascular endothelium,decrease peripheral resistance, & reduce myocardial oxygen demand .Itmarkedly relax arterioles and milder effects on veins. They do not compromisehaemodynamics and cerebral and renal perfusion.

a.) Telmisartan
Absorption: Absorbedorally, bioavailability is about 42%
Distribution: Itis distributed in to the body in a highly protein bound form.
Metabolism: Metabolizedby glucuronide conjugation in to an inactive metabolite.
Excretion: Excretedmainly through faeces and rest is excreted through urine.
b.) Amlodipine
Absorption: Wellabsorbed orally.
Distribution: Widelydistributed in a protein bound form.
Metabolism: Metabolizedin liver, but there is no active metabolite
INDICATIONS
ACE inhibitors: Thiazide diuretics:Potentiates effect of these drugs.
Beta Blockers : Best avoided, especially by those withimpaired LV function due to depressant effect on myocardial contractibility
or AV conduction.
Fentanyl: Severe hypotension orincreased fluid volume requirements.
